The last time a policy changed, do we actually know who went back and re-confirmed it - or do we only know who confirmed the version before?

Publishing a policy is not the same as proving anyone read it. Most organizations can show a document went out; almost none can show, by name or by department, who actually confirmed they saw the current version - and fewer still can show it again after that policy changes.

Employees are entitled to protections across a wide span of law - equal pay, maternity and parental protections, safe working hours, protection from child labour and forced labour - and when someone asks “how do we know our people actually know their rights,” most organizations can point to a policy folder, not a record.
